Mushroom resembling a witch's hat, can be found in Forests

Note that the mushrooms you gather are poisoned before you cook them. They will drain your maximum humours by up to 20%, reducing over time (the debuff from one witch's hat can take over half an hour to wear off completely). This poison can be removed through boiling in a Cauldron, Copper Pot, or Clay Pot. Aside from Witch's hat, only Waxing Toadstool and Waning Toadstool are poisonous. All other mushrooms are safe as of 14/06/2013.

The tooltip will note when the mushroom has been boiled (and is thus poison-free).
