A backpack gives you 8 more inventory slots which are accessed when you equip it and right click on it.
All items in your backpack will survive even after getting knocked out and teleporting to Boston, be sure to keep your most important items here.
When you are using hotkeys like shift+right click, items from backpack are not managed.
When crafting, the game will not recognize items in your backpack for use as inputs/ingredients available.
You'd have to move them to your main inventory.
You can use this to you advantage if you want to keep something like Aged Driftwood safe from wood-working crafting which might inadvertently use your Aged Driftwood instead of a Woodblock if you kept them in your main inventory.
